There are 20,000 premature babies in Taiwan every year. Mothers and children still have to face slow recovery and arduous growth journey in the future. Doctors remind that the incidence of cervical incompetence is about 1 in 500 to 1 in 2,000. It is one of the high risk factors for premature birth, but as long as this test is added during prenatal check-up, this risk can be easily avoided.
Cathay Hospital held a "Papal Fairy" event yesterday for premature babies to return to their parents' homes. Many parents took their children to participate. A pair of lively and cute twins, Qiqi and Haohao, were born at 25 weeks. They weighed only 877 grams and 840 grams. They were very dangerous premature babies.
When the mother of twins had a high-level ultrasound at 21 weeks of pregnancy, she found that the "cervix was incompetent" and had dilated two fingers. The fetus was about to slide into the birth canal. She was quickly hospitalized for tocolysis and underwent cervical cerclage. Because of the cervical cerclage, The baby stayed in the womb for five more weeks to grow up, and the mother was given time to administer steroids to promote the maturation of the fetal lungs. Later, at almost 26 weeks, the amniotic membrane ruptured, so the twins had to be born early.
The probability of occurrence is at most 1 in 500
Hong Yili, director of the Neonatal Intensive Care Unit of Cathay Hospital, said that the probability of cervical atresia is about 1 in 500 to 1 in 2,000, so it is recommended to do a cervix scan during prenatal check-ups between 20 and 24 weeks of pregnancy. If a problem is found during the length check, surgery can improve it and significantly avoid the chance of premature birth.
Cai Mingsong, director of the obstetrics and gynecology department of Cathay Hospital when she was 20 to 24 weeks pregnant, said that "cervical insufficiency" refers to the spontaneous and painless shortening of the cervix and the dilation of the cervix without any symptoms of uterine contraction. In severe cases, the amniotic membrane will bulge into the vagina through the open cervix, and there will be a risk of premature delivery of the fetus.
Cai Mingsong said that because pregnant women usually have no symptoms, many women do not find out that the cervix has opened until their water breaks or they have a foreign body sensation in the vagina and seek medical attention. If the amniotic sac has bulged, it is almost difficult to save the child.
Cai Mingsong reminded that anyone with congenital uterine abnormalities, cervical lesions, resection surgery, multiple abortion surgeries, multiple births, etc., is a high-risk group for cervical insufficiency, and the cervical length should be measured in the second trimester. Measurement.
When cervical insufficiency is found, cervical cerclage surgery can be considered, and antibiotics and appropriate tocolytic drugs can be given. Early treatment can effectively extend the pregnancy weeks, increase fetal survival rate and reduce complications.
